Application Form

For the process of acquiring an unabridged birth certificate, an application form must be filled and submitted to the Department of Home Affairs.

A Complete Form BI- 154 must be submitted to DHA South Africa

Late Birth Registration

According to Births and Deaths registration act 1992, all South African citizens must register the birth of their child within the first 30 days.
